Appointment Requests and Communications

Submitting a contact form or appointment request through the Site does not confirm an appointment. All appointments must be confirmed by our office directly.

Do not use the Site to communicate urgent or emergency matters. Form submissions are reviewed during business hours only. If you have a medical emergency, call 911.

The Site's contact form is not a secure channel. Please do not submit detailed health information, insurance identification numbers, Social Security numbers, or payment information through it.
